Methylation Panel

A precision look at how your body activates vitamins, detoxifies, and repairs

Methylation is a core biochemical process that turns genes on and off, recycles homocysteine, builds neurotransmitters, supports detox pathways, and keeps inflammation in check. When methylation is sluggish or imbalanced, you can feel it as low energy, brain fog, mood swings, poor stress tolerance, slow recovery, and plateaus in body composition. The Methylation Panel helps us see where the traffic is backed up so we can clear the road and get you moving again.

What This Test Is

The Methylation Panel measures key metabolites in your methylation and transsulfuration pathways using plasma. Most analytes are run by LC-MS/MS for high sensitivity. A cheek-swab genetics add-on can be included to assess SNPs that influence enzyme efficiency. Results are mapped onto an easy-to-read pathway graphic to show where support is needed.

Core biomarkers commonly included

SAM, SAH, SAM/SAH ratio, methionine, homocysteine, cystathionine, cysteine, glutathione, betaine, choline, dimethylglycine, sarcosine, serine, taurine, and interpretation ratios such as methylation and sulfur balance.

Sample types

Plasma for metabolites, with an optional buccal swab if you choose the Methylation Genomics add-on.

What Your Results Show

You will receive:

  • Pathway map with green, yellow, or red zones to highlight bottlenecks
  • Key ratios such as SAM/SAH that reflect global methylation potential
  • Transsulfuration status to gauge sulfur amino acid flow and glutathione demand

Optional genomics insights for enzymes like MTHFR, MTRR, COMT, and SHMT1 to explain predispositions that may raise your nutrient needs under stress, illness, or high training loads.

Who Benefits

  • Persistent fatigue, brain fog, or subpar recovery despite “normal” labs
  • Mood, focus, or stress regulation concerns
  • Estrogen metabolism or PMS symptoms
  • Elevated homocysteine or family history of cardiometabolic disease
  • Longevity programs, preconception planning, or optimization during weight loss and performance blocks

How We Use The data At Peak by MD

We pair your methylation findings with your intake, bloodwork, PNOĒ RMR and VO₂ testing, and body composition to build a precise plan. That plan may include:

  • Food framework rich in methyl donors and sulfur amino acids, with alcohol and toxin-load adjustments
  • Smart supplementation using activated folate, methylcobalamin or adenosyl/hydroxy B12, P5P, riboflavin, magnesium, betaine or choline, NAC or glycine for glutathione support, and targeted antioxidants
  • Timing strategies that align nutrients with training, recovery, and sleep
  • If genomics are added, we tailor dosages and forms to your enzyme profile and retest selected markers to confirm progress

What Is Included

  • Phlebotomy for plasma methylation metabolites
  • Optional cheek-swab Methylation Genomics add-on
  • Physician interpretation and a written action plan with nutrition, supplement, and lifestyle steps
  • Integration with your broader program so recommendations convert into daily wins

Preparation And Logistics

  • Fasting is often preferred for metabolic clarity unless otherwise advised
  • Continue prescription medications unless your clinician instructs you to pause
  • Most patients complete the blood draw in our office. If adding genomics, we collect your cheek swab on the same visit. The test uses plasma for metabolites, and a buccal swab only if you choose genetics.

Frequently Asked Questions

No. The metabolite panel shows current pathway function. The optional genomics page shows predispositions. Using both offers the most complete picture.

Genova uses LC-MS/MS for most analytes, which provides high analytical sensitivity for small molecules in these pathways.

We usually retest targeted markers three to six months after an intervention to confirm that homocysteine, SAM/SAH balance, and glutathione status have improved.

Coverage varies. We can provide the documentation you need, and we will discuss the most cost-effective approach for your goals.
